Client Experience Survey

In an effort to improve the quality of our services, we need your feedback! Your opinion is critical to make sure we provide you with the right services you need.

The survey will take 5 to 10 minutes to complete and all the information you include will be kept strictly confidential. To ensure we gather the most effective data possible, please be sure to fill out the survey completely and check your answers once you have finished.

Currently, we are only completing the Client Experience Survey in-person. Please only complete the survey once.

1. In-person

To obtain a physical copy of the client experience survey, copies are available at both medical clinics (Eccles and Rosemount Branch). We are glad to assist you if you need help completing the survey.
