Our Programs and Services

Somerset West Community Health Centre offers a wide variety of programs and services designed to meet the needs of our community. We offer services in Primary Care, Health Promotion, Families and Children, Mental Health and Addictions as well as Chronic Disease Management.     Our in-person sessions are paused until further notice. 

    Learn everything you need to know about feeding your baby from 6 to 12 months. This workshop is run by our Community Dietitian and covers a variety of topics including:
    • When and how to first introduce foods to your baby
    • Key nutrients for growing babies
    • Tips for healthy eating habits for life and avoiding ‘picky eating’
    • Learn how to make your own baby food
    • Updated allergy guidelines
    • Take home recipes, handouts and taste samples of homemade baby food

    In order to respond to the COVID-19 pandemic, we have cancelled all group sessions until further notice. 


    This six-week program offers expecting parents information in Mandarin on healthy pregnancy, labour and delivery, newborn care and breastfeeding, and community resources for pre and post-natal care. Offered by an Ottawa Public Health public health nurse four times per year.

    In order to respond to the COVID-19 pandemic, Yet Keen Seniors’ Day Centre will pause its regular in-person programming for the safety and concerns of a vulnerable population. To continue to serve the Yet Keen community, the staff team will be reaching out to members by phone to identify individual needs and provide information, support and referral remotely. We recognize that closing the Centre will be difficult for some of our members and caregivers. We encourage members to call the Centre at (613) 232-6695 during office hours (Tuesday to Friday, 9:30am-3:30pm) should you have any questions or concerns.

    The Ottawa Good Food Box is a non-profit, community-based initiative bringing neighbours together to buy a variety of delicious and nutritious fresh fruits and vegetables at wholesale prices. The goal is to purchase food that is in season and is grown as close to home as possible. Working as a large buying club with centralized buying and coordination, the Good Food Box is open to everyone – no membership fees!

We have temporarily closed the NESI Drop-in space and shifted our focus to outreach and van services.  We have gear distribution outside of 55 Eccles Monday - Sunday from 10:00 am - 4:00 pm, or you can contact the NESI van at 613-761-0003.


    NESI offers harm reduction services for people who use drugs, including safer inhalation and injection supplies, collection and disposal of used equipment, and peer support in a non-judgmental environment.

    Stop by the NESI drop-in room at the Eccles Branch or call the NESI van to have supplies delivered!

    Located at 299 Rochester Street, the Rochester Heights Community House offers a variety of educational, recreational, and social programs for residents of the Somerset West Service Area

    Free programming includes after-school programs for children, community kitchens for adults, and summer camps and leadership programs for youth. Through programming and one-on-one support, we empower people to develop leadership and practical life skills to improve their health and well-being. We listen, suggest community resources and services, and identify unmet community needs and advocate for them.

    Services are available in English and Spanish.

    In order to respond to the COVID-19 pandemic, we have cancelled all group sessions until further notice. 


    This class is intended for people living with a chronic disease in the Somerset West Community Health Centre service area who are new to Tai chi and Qigong.

    Combining meditation and movement to improve and maintain health, Tai Chi and Qigong consists of a series of movements done either very slowly or quickly to help move the body's chi. The benefits of Tai chi and Qigong include: 

    • Reduced pain and stiffness
    • Improved mood
    • Enhanced immune function 
    • Lower blood pressure

    NESI offers harm reduction services for people who use drugs, including safer inhalation and injection supplies, collection and disposal of used equipment, and peer support in a non-judgmental environment.

    Complex Respiratory Care Program

    The Complex Respiratory Care Program is a community based program which helps clients with complex respiratory issues integrate back into the community from both the acute and sub acute care setting. Service is provided by a Registered Respiratory Therapist.

     

    The Complex Respiratory Care Program provides assessment, in home trach tube changes, training and education to medically stable clients with tracheotomy or ventilator-assisted individuals and their families using a variety of tools. The respiratory therapist provides routine follow up and ongoing support to the client while in the community. The respiratory therapist will work with the discharging facility to promote a safe and successful discharge to home or community living.

     

    Education is offered on an individual basis or in a group workshop to clients, caregivers, health care professionals and family members. Individualized training and education is available for invasive ventilation, non-invasive ventilation, mouthpiece ventilation, lung volume recruitment, cough assist, tracheostomy tubes, suctioning, and respiratory assessments.

     

    Clinical support and education will be offered to clients who meet the following criteria:

     

    1)      Medically Stable with a primary care physician and/or respirologist

    2)      Adult population

    3)      Invasively/Non-invasively ventilated at home or community living

    4)      Invasively/Non-invasively ventilated pending discharge to home or community

    5)      Tracheostomy tube and routine trach tube changes

    6)      Use of Cough Assist device

    7)      Champlain LHIN

     

    The cost of the service is covered for clients with a valid Ontario Health Insurance Plan (OHIP) card living within the Champlain LHIN.

    The Ottawa Community Lung Heath Program (OCLHP) provides an evidence based approach that improves the health and quality of life of people living with Chronic Obstructive Pulmonary Disease (COPD). The OCLHP consists of assessment, spirometry (breathing test), education, the development of an action plan and follow-up care for clients with COPD . When someone is referred to the lung health program, they will be seen by a Registered Respiratory Therapist/Certified Respiratory Educator (CRE). The CRE will help you understand and manage your symptoms.

    The OCLHP also provides a community based Pulmonary Rehab (PR)Program.

    This 8-week pulmonary rehabilitation program is designed for people with COPD (chronic obstructive pulmonary disease) to decrease symptoms and improve their ability to function in daily life.

    The program consists of exercise, education and support delivered in: an 8-week training program (2 sessions per week) and an optional maintenance phase (1 session per week). Clients are followed by a Certified Respiratory Educator (CRE) and supervised by a personal trainer while exercising.

    Ottawa Language Access Social Enterprise offers support to health care organizations and providers who are looking to provide linguistic support to their clients. We are committed to provide professional interpretation support to ensure that individuals can communicate with their health care providers in their language of choice.

    Lactation consultants are professional breastfeeding specialists trained to teach mothers how to feed their baby. They:

    • Help women who experience problems breastfeeding, such as painful nursing, latching difficulties, and low milk production
    • Lend women breastfeeding equipment (like breast pumps) and teach them how to safely store expressed breast milk
    • Help women who wish to continue breastfeeding after returning to work or school

    What to Expect at Delivery

    The family physicians at SWCHC deliver babies at The Ottawa Hospital, Civic Campus on Carling Avenue. At the Civic Hospital we are family centered and offer a comfortable birth atmosphere for the low risk patient.

    When you think you might be in labour, go to the Civic Hospital 4th floor Birthing Unit triage area. If you are unsure if it is time to go to the hospital or you have any questions, you can telephone a labour and delivery nurse at 613-761-5112. When you arrive at triage, a nurse and family medicine resident will assess you. The family physician on call that day will then be contacted and will discuss what is happening with your labour or other pregnancy issues.

    Patients in active labour can expect one on one nursing care and the involvement of a family medicine resident. The delivery rooms offer Jacuzzi tubs, rocking chairs and birthing balls. We encourage women to labour in a variety of positions.

    If at any time your pregnancy becomes high risk we have 24 hour access to obstetricians, pediatricians, neonatal special care and anesthesia. After the baby is born s/he will "room in" with Mom; Dad/partner are welcome to stay too. Keeping baby skin to skin and in the same room as Mom improves bonding and breastfeeding.

    Congratulations on your pregnancy! The SWCHC pregnancy team is privilege that you have chosen us to be part of this most exciting time for you and your family. Once registered with us all pregnant patients are assigned to one of our Pregnancy Team physicians. Prenatal appointments are offered weekdays as well as Monday, Tuesday, Wednesday and Thursday evenings.

    You will see a physician or physician assistant at each prenatal check-up, and possibly other members of the  team. At each visit we will assess the health of both you and your baby. We take time to address any concerns you may have, and provide education about pregnancy. Information is also available on prenatal classes, labour support, birth planning, and staying healthy during pregnancy.

    Your initial obstetric appointment is with Bonnie, our Prenatal Point Person. She is also a Registered Dietitian and lactation consultant, and coordinates our obstetrics program.

    Please note that all our physicians on the Pregnancy Team are female, however, you may be seen and examined in our clinic or hospital by other team members who are male.

    HIV/AIDS in Ontario

    Who is most at risk of contracting HIV/AIDS?

    Communities who are at most risk of contracting HIV/AIDS in Ontario are:

    What are high risk activities?

    Participating in the following activities puts you at a higher risk of contracting HIV/AIDS: 

    1. Unprotected vaginal and/or anal intercourse (including condom breakage, dipping and delayed application of condom) with either:
      • An HIV positive partner with a detectable viral load; or
      • A partner of unknown status (unless both partners have been mutually monogamous for more than 3 months, and have both been tested HIV negative).
    2.  Injection drug and crack use, especially with shared equipment.

    Should I get tested?

    Yes, if you belong to one of the communities listed above or have engaged in a high risk activity.

    When should I get tested?

    Following a high risk exposure to HIV, test as soon as possible if you have symptoms (fever, swollen lymph nodes, night sweats, weight loss, rash, oral yeast infection).

    If you have no symptoms, get tested 3 weeks after you engaged in the high risk activity. This is recommended because of the window period for HIV testing.

    If you have engaged in multiple high risk activities, time your tests based on the date of your most recent high risk encounter.

    What is the "window period" for HIV testing?

    The “window period” is the time between when a high risk activity occurred (i.e. when you may have been exposed to HIV) and when an HIV test can give an accurate result. This period exists because HIV antibodies must be present in the blood for the test to accurately detect an HIV infection, and these antibodies develop over time. The window period is between 3 weeks to 3 months after engaging in a high risk activity. During the window period, it is possible that you may have been infected with HIV and that the test will give a negative result because it is unable to detect the infection.

    Primary Care Services

    Doctors, nurses, dietitians and more offer a range of medical services on an appointment basis at both the Eccles and Rosemount branches. To become a registered client with one of our physicians or nurse practitioners, people must live in the Somerset West Community Health Centre service area.

    Our primary care practice is currently closed. We encourage clients to continue looking for a provider elsewhere to find someone as soon as possible. Persons living in our catchment area can certainly come to our walk-in clinic where they will be seen by a nurse practitioner (NP) for any health concerns as well as use any of our services.

    After Hours Care

    When the clinic is closed, we have a physician on-call for registered clients of  our Primary Care Services with urgent health matters. To contact the on-call physician, call 613-238-1220 ext. 1.
     
    For medical emergencies, call 9-1-1.

    Our walk-in services are currently offered through virtual care available by phone and video appointment. Appointments are available Monday to Friday from 1-3 PM. To book a same day appointment, please call us at 613-238-1220.

    If you do not have reliable access to a phone, in-person same day appointments for primary care are available at our 55 Eccles St location Monday to Friday 3:00 p.m. - 4:00 p.m. 

    Beginning July 27, we will be expanding our virtual walk-in hours. The new hours will be Monday, Tuesday, Wednesday & Friday 9:00 a.m. - 4:00 p.m., Thursdays from 1:00 p.m. - 4:00 p.m. This schedule will be in place until September 25. 


    Somerset West Community Health Centre has two walk-in clinics that operate Monday to Friday. Nurse practitioners provide assessment, treatment and referrals for people with immediate health issues.

    The clinics are for anyone living in the Somerset West Community Health Centre service area, including those without an Ontario health card and/or a permanent address.

    *Please note we may close our walk-in earlier than our posted hours due to:

    • Heavy client volume
    • Medical emergencies 
    • Staff shortages

Come have fun discussing new recipes and adapting old favourites to help manage diabetes, high cholesterol, high blood pressure and other health issues! For seniors 60 years and older.     Treatment options such as the following are also available to people who access the SCS:

    • Immediate access to individual and group counselling
