Here are different ways to provide feedback:
Please provide all your contact information including a phone number in the form at the bottom of this page and we will be in touch with you within five working days.
2. With Your Provider
If you are comfortable doing so, discuss your concerns with your provider. They know best and are often able to resolve your issue right away. You may also ask to speak to the manager or coordinator of the program or service for help.
3. Client Complaint Form
Client Complaint Forms are available at the reception areas of the Eccles Branch and the Rosemount Branch. All complaint forms will be sent to the Executive Director, Program Manager and staff involved. We will contact you within five working days of receiving your written complaint.
